Health & Wellbeing Checks that help you stay where you belong

Proactive, preventative support for adults aged 18+ — no ongoing care package needed.

At Bluebird Care Merton, quality care is more than a list of tasks. It’s noticing the small changes early, so the people we support can stay where life feels right: at the kitchen table they know, looking out at a garden they love.

Our Health & Wellbeing Checks are built for that kind of everyday confidence.
You don’t need to be a Bluebird Care customer to use this service — anyone aged 18 and over is welcome.

What’s included

Each check is carried out by a trained member of our team. For customers receiving live-in care, this team member is supported by one of our dedicated Wellbeing Specialists, who provides regular observations and feedback to ensure a more detailed picture of ongoing health.

Checks can include:
• Blood pressure monitoring
• Temperature readings
• Pulse and oxygen saturation
• Urinary Tract Infection (UTI) analysis
• Nutrition and hydration monitoring
• Weight checks and management
• General wellbeing observations

All readings are taken using Bluetooth-enabled medical devices and recorded on a secure tablet. The system calculates a NEWS2 score (National Early Warning Score 2) — a clinically recognised NHS tool that helps spot early signs of deterioration across several vital signs. In short: it turns day-to-day numbers into timely insight.
You’ll also receive clear charts and graphs, so patterns and changes are easy to track over time.

What is NEWS2?
NEWS2 is a simple scoring system used across the NHS to flag when someone may need closer monitoring or medical review. It looks at breathing, blood pressure, pulse, temperature and more. Higher scores prompt faster action. We use it because it’s proven, consistent, and puts safety first.

More than numbers
Our health monitoring tablet is more than a digital checklist. It also lets us:
• Log any recent falls
• Record wellbeing notes and clinical concerns
• Keep a secure, regularly updated profile for each person (GDPR compliant)

With consent, we can share accurate updates with GPs and other professionals — saving time and helping the right support to arrive sooner.
We also work alongside trusted partners such as Outside Clinic (for eye care) and Wiltshire Farm Foods (for nutritious, easy-to-heat meals), so practical follow-up is simple when it’s needed.

Real results, real reassurance
One of our customers, who is mostly bedbound after a long illness, seemed a little “not herself” during a routine visit. Because her Health & Wellbeing Check includes a UTI test as standard, we picked up early signs of infection. The results were uploaded instantly, shared with her GP, and a prescription was arranged the same day — no hospital trip, no long wait. She recovered at home, safely and quickly.

We see the same impact after hospital stays: regular checks highlight low hydration, sudden blood pressure drops, or unexpected weight changes — small things that, when caught early, can prevent bigger problems.

Who is it for?
• Starting care with Bluebird Care Merton — a complimentary baseline Health & Wellbeing Check in the first week of your care, so we begin with a clear picture and the right plan.
• Annual reviews — a complimentary Health & Wellbeing Check as part of your 6-monthly/yearly review, to refresh baselines and adjust support where it matters.
• Customers receiving 24-hour live-in care — free weekly Health & Wellbeing Checks are included, with weekly UTI checks as part of our routine.
• People coming home from hospital — two weeks of free checks to support recovery and reduce the risk of readmission, with a small top-up fee if you’d like to continue.
• Anyone aged 18+ — available as a standalone service, even if you don’t receive ongoing care from us.
Why families choose us
• Person-centred. We tailor checks to each person’s needs and conditions.
• Proactive. We look for patterns before problems escalate.
• Professional. Trained staff using NHS-recognised tools, including NEWS2.
• Connected. With your consent, we share clear information with GPs and community teams.
• Dementia-aware. We support the Alzheimer’s Society and share our learning, so our approach is calm, respectful and informed.
• Accessible. Adults 18+ can use this as a standalone service — no ongoing care package required.

Most of all, families tell us they feel peace of mind. GPs value the accurate reporting. And the people we support feel seen, not rushed.
